Report Suspected Abuse and Neglect
According to the Minnesota Department of Health, and Department of Health DHS to report abuse contact the Minnesota Adult Abuse Reporting Center (MAARC) to file a confidential report against a nursing home, assisted living, memory care or other elder health care provider at 1-844-880-1574. The MAARC is the statewide common entry point for to report suspected maltreatment of vulnerable adults. Reports will be promptly submitted to the appropriate investigative agencies.
In addition to filing a report with the MDH you can contact the Minnesota Ombudsman for Longterm care to assist with day-to-day care issues for residents. The state ombudsman can act as an intermediary between the facility and the resident and their family to assist with getting the assistance and care needed. The ombudsman can be reached at 1-800-657-3591.

According to the DHS site, it Minnesota’s policy is to provide safe environments and protective services for vulnerable adults and children. If you suspect someone is being abused, report the abuse now. There is a link for Adult abuse (report the abuse of a vulnerable adult) and Domestic abuse (report domestic violence and abuse).
