In a report from the Minnesota Department of Health it is alleged that a client at Walker Methodist Westwood Ridge II when the facility failed to provide adequate assessment and follow-up after resident had a fall. Fall resulted in a femur fracture.

Neglect was substantiated. Neglect occurred when the resident fell during an unwitnessed fall, had unimproved pain with decline in his/her mobility., and the facility’s nursing and medical staff did not notify the resident’s orthopedic physician as instructed per the hospital discharge order.
