REM Woodvale Albert Lea Complaint Findings for Exploitation
In a report concluded on December 26, 2012, the Minnesota Department of Health cites REM Woodvale Albert Lea for exploitation by staff.
The allegation is financial exploitation based on the following: the Alleged Perpetrator (AP) stole money from seven facility clients.
REM Woodvale Albert Lea Complaint Findings for Physical Abuse and Skin Assessment
In a report concluded on September 14, 2012, the Minnesota Department of Health cites REM Woodvale Albert Lea for physical abuse by staff – neglect of health care.
It is alleged that Client #1’s rights were violated based on the following: Client #1 was observed restraining in a wheelchair with a seatbelt and with a chair in front of him, after he had been observed pacing the hall. Client #1 does not have a behavior management plan that includes physical restraints.
The allegation is neglect based on the following: Client #2 and Client #3 are not being provided with adequate care in accordance with their needs. Client #2, who is incontinent and has open skin area, is not checked and changed during the night shift. On at least one occasion, Client #3 was observed sitting up in the wheelchair during the night shift and is not repositioned or taken to the bathroom
