In a report concluded on July 12, 2011, the Minnesota Department of Health cites Owatonna Care Center for neglect of health care – fall.

The allegation is neglect based on the following: Resident #1 was not adequately supervised, exited the facility and fell resulting in injuries.

In a report concluded on June 23, 2011, the Minnesota Department of Health cites Owatonna Care Center for neglect of health care – decubitus ulcer.

The allegation is neglect based on the following: Resident #1 was admitted to the hospital unresponsive, with low blood pressure, sepsis, an open wound on his hip, stage II decubitus on his coccyx, and multiple other skin concerns.
