
MDH Cites Oak Terrace Senior Housing After Resident Fall
In a report from the Minnesota Department of Health it is alleged that a client at Oak Terrace Senior Housing in LeSuer was neglected when facility staff failed to provide adequate supervision, resulting in a pubic fracture.

Neglect was substantiated. The client fell and acquired a pelvic fracture. The home care provider was unable to produce documentation indicating the client received the required safety checks as required by the client’s care plan.
