It is alleged that abuse occurred when a staff person/alleged perpetrator (AP) entered the rooms of Client #1, Client #2, and Client #3 and inappropriately touched and kissed them.

In a report concluded on April 19, 2011, the Minnesota Department of Health cites Madonna Meadows Rochester for neglect of health care – falls.
The allegation is neglect based on the following: a staff person, alleged perpetrator (AP) left client #1 standing unattended in the bathroom, which resulted in a fall with injury. The client’s care plan stated she needs one staff assist with all transfers and a gait belt is required when walking or transferring.

The allegation is abuse based on the following: two clients (Client #1 and Client #2) were sexually assaulted by a staff person, the alleged perpetrator (AP).
