If you want to file a investigate the licensure, look up a provider’s past history of deficiencies, or file a complaint you can do so through links available on the MDH website or by printing out the forms attached at the site and sending them by mail, or by calling . There is also a nursing home report card available which grades the facilities bases on several quality and care factors.
