Evergreen Terrace Grand Rapids Complaint Findings for Physical Abuse
In a report concluded on August 16, 2012, the Minnesota Department of Health cites Evergreen Terrace Grand Rapids for physical abuse by staff.
The allegation is abuse based on the following: a nursing assistant, alleged perpetrator (AP) slapped a resident in response to the resident hitting the AP.
