MDH Cites Augustana Emerald Crest Minnetonka After Resident Suffers Femur Fracture

In a report from the Minnesota Department of Health it is alleged that a client at Augustana Emerald Crest Minnetonka was neglected when the alleged perpetrator (AP) failed to supervise the client; the AP was sleeping during the night shift. The client fell and sustained a left femur fracture. The client died 19 days later.

Failure to Supervise Leads to Preventable Fall

Neglect was substantiated. The alleged perpetrator (AP) was responsible for the maltreatment. The client was known to wake frequently during the night and required supervision. The client was known to wake frequently during the night and required supervision, engagement, and redirection when awake. The client began wandering in the common area, fell sustained a left femur fracture, and later died. The AP assigned to her care was sleeping while on duty and failed to supervise or engage the client as directed in the client’s service plan.
