Ebenezer Care Center Minneapolis Complaint Findings for Exploitation
In a report concluded on August 16, 2014, the Minnesota Department of Health cites Ebenezer Care Center Minneapolis for exploitation by staff.
It is alleged that a resident was financially exploited when a staff, alleged perpetrator (AP) used the resident’s credit care and made unauthorized charges.
Ebenezer Care Center Minneapolis – Theft from Credit Card
Based on a preponderance of evidence, the allegation of financial exploitation is substantiated. The alleged perpetrator (AP) admitted to police to making unauthorized charges using the resident’s bank card for their own personal use. The AP was charged with financial transaction card fraud.
A police report identified the AP and confirmed that the AP admitted to making multiple unauthorized charges using the resident’s bank card.

Ebenezer Care Center Minneapolis Complaint Findings for Nursing Care
In another report concluded on March 15, 2011, the Minnesota Department of Health cites Ebenezer Care Center Minneapolis for nursing care.
It is alleged that Resident #1’s right to receive care in accordance with acceptable nursing standards was violated based on the following: staff did not provide the resident with adequate clothing for the cold weather to go to a physicians appointment. Also she was not provided with leg supports on her wheelchair causing her feet to drag on the floor when the chair was pushed. In addition, the resident’s room is cold.
