In a report from the MDH it was alleged that there was neglect of a client at Chris Jensen Health in Duluth when staff failed to perform CPR when the VA was found unresponsive. The VA’s order was full code. The VA passed away.

Based on a preponderance of the evidence neglect occurred when a resident who requested cardiopulmonary resuscitation (CPR) did not receive CPR from four nurses who responded when the resident had no pulse or respirations.

Chris Jensen Health & Rehab Center Complaint Findings for Exploitation

In a report concluded on July 2, 2012, the Minnesota Department of Health cites Chris Jensen Health & Rehab Center for exploitation by staff.

The allegation is financial exploitation based on the following: the alleged perpetrator (AP) took $100.00 from the resident with instructions to purchase specific clothing items.  The AP purchased gas for her car and returned a portion of the money to the resident.

In another report concluded on June 20, 2012, the Minnesota Department of Health cites Chris Jensen Health & Rehab Center for Physical Abuse by staff.

The allegation is abuse based on the following: a nursing assistant/alleged perpetrator (AP) slapped the resident on her face, spit in her face, held on to her in an aggressive manner and told the resident to grow up and stop acting like a two year old during the resident’s bath.
