Bethesda Lutheran Community Cottage 5 Coon Rapids in City Complaint Findings for Abuse
In a report concluded on July 31, 2014, the Minnesota Department of Health cites Bethesda Lutheran Community Cottage 5 Coon Rapids for abuse – physical staff.
It is alleged that a client was restrained by two staff, Alleged Perpetrator 1 (AP1) and Alleged Perpetrator 2 (AP2), in an unsafe manner in accordance with facility policy.
Based on a preponderance of evidence abuse occurred when AP1 and AP2 restrained a client in a prone position, face-down on the floor, applied physical pressure to the client’s back, and manually restrained the client’s legs, AP1 and AP2 implemented an unsafe restraint procedure which placed the client at significant risk for potential injury.
The client sustained a 2-inch by 2-inch abrasion on the left shin when s/he bumped into a wheelchair and a bruise (size not documented) on the left temple when s/he bumped into the door frame. Both these injuries were sustained during the client’s behavioral outburst, prior to the incident of physical restraint by AP1 and AP2. The nurse assessed the client’s injuries the next day, which were minor and quickly resolved.
