In a report concluded on April 19, 2011, the Minnesota Department of Health cites Avera Morningside Heights Marshall for neglect of health care – falls.
The allegation is neglect based on the following: Resident fell when the alleged perpetrator (AP) left Resident standing unassisted. The fall resulted in a fracture of resident’s elbow.
