Failure to Respond to Significant Weight Loss at Traditions Owatonna II in Owatonna Minnesota

Traditions Owatonna II Owatonna Complaint Findings for Neglect

In a report concluded on October 3, 2013, the Minnesota Department of Health cites Traditions Owatonna II for neglect of health care.

Based on a preponderance of evidence, neglect occurred when the client experienced significant unplanned weight loss and the facility did not implement necessary interventions to address the weight loss.  As a result, medical intervention and treatment was delayed and the client continued with progressive weight loss throughout the service agreement period.

In another report concluded on January 13, 2011, the Minnesota Department of Health cites Traditions Owatonna II for neglect of health care.

The allegation is neglect based on the following: a client has experienced multiple falls with injuries while supervised.
