In a report concluded on February 17, 2015, the Minnesota Department of Health cites Parkview Care Center Wells for violation of patient rights.

Substantiated Complaint Against Parkview Care Center Wells

Based on preponderance of evidence neglect did occur.  The facility staff stated that resident #1 on psychoactive medications without justification and then failed to monitor the resident to ensure the resident’s health was maintained.  As a result, the resident exhibited extreme agitation and sedation after the initiation of the medications.  This change in the resident’s condition contributed to the resident’s inability to communicate chew food adequately, and maintain continence of bowel and bladder function.
