In a report concluded on June 2, 2011, the Minnesota Department of Health cites Golden Living Center Lynwood Fridley Anoka County for neglect of health care.

Failure to Provide Oxygen – Golden Living Center Lynwood Fridley

The allegation is neglect based on the following: Resident #1 was not provided with adequate nursing care and monitoring when she was sent to a scheduled clinic appointment with a temperature of 102 degrees, an empty oxygen tank, with her stage 2 pressure ulcer covered in feces with no dressing and her old PICC line access site was draining.  In addition, the resident was still wearing her hospital wrist band which identified her as “DNR” although this was no longer her wishes.  The resident was sent to the hospital and diagnosed with Sepis.
