In a report concluded on April 17, 2014, the Minnesota Department of Health cites Frazee Care Center Frazee for neglect to health care.

Unanswered Ventilator Alarm Leads to Neglect Complaint at Frazee Care Center

It is alleged that neglect occurred when a resident’s ventilator alarm was heard ringing and staff did not respond in a timely manner.  The alarm rang between five and ten minutes and when checked, the resident was found to be deceased.
