Cherrywood Advanced Living in Sauk Rapids Cited for Neglect After Resident Left Unattended in Specialized BRODA Chair Falls

In a report concluded on August 6, 2015, the Minnesota Department of Health cites Cherrywood Advanced Living in Sauk Rapids after allegations that a client was neglected when a staff failed to follow the care plan and the client fell our of his/her wheelchair.  The client was not assessed and sent to the hospital until the next day and subsequently died.

Based on the preponderance of the evidence the MDH substantiated neglect against Cherrywood Advanced Living.  The facility failed to provide the necessary care and services to evaluate, assess, develop and plan care, and train staff on the client’s use of a BRODA chair (a high back wheelchair that can be adjusted to multiple).  The client fell from the BRODA chair, sustained a femur fracture, and passed away.
