Birchwood Health Care Center Forest Lake Complaint Findings for  Exploitation

In a report concluded on November 21, 2012, the Minnesota Department of Health cites Birchwood Health Care Center Forest Lake for exploitation by staff.

It is alleged that a client was financially exploited when a staff took the client’s Fentanyl patch for her/his own use.  Medications were also missing from two other clients.

Birchwood Health Care Center Forest Lake in City Complaint Findings for Neglect

In another report concluded on June 10, 2011, the Minnesota Department of Health cites Birchwood Health Care Center Forest Lake for neglect of health care – falls.

The allegation is neglect based on the following: Resident #1 was dropped from a hoyer lift while being transferred by two staff.  The resident sustained bruises and a golf ball sized contusion on her forehead.
