In a report concluded on June 1, 2011, the Minnesota Department of Health cites Bethany Home for neglect of health care – medications.

Resident Given Wrong Dose of Medication – Bethany Home Alexandria

The allegation is neglect is based on the following: Resident #1’s medication orders were transcribed inaccurately and as a result she was given the wrong dosage of Potassium.  This resulted in her developing a high Potassium level which necessitated her being sent to the hospital where she later expired.
