Augustana Healthcare Center Minneapolis Allegation of Physical Abuse

In a report from the Minnesota Department of Health dated January 23, 2017, it was alleged that a resident at Augustana Healthcare Center Minneapolis was abused when the alleged perpetrator (AP) physically abused the resident by slapping the resident.

Based on a preponderance of the evidence, abuse occurred. Staff witnessed the alleged perpetrator (AP) slap the resident.

The resident had diagnosed dementia with behaviors.  The resident had a care plan for staff to provide a consistent calm environment and remove stimulation from the area when the resident was upset.

On morning, the resident was sitting at a table in the dining room and became upset about a spill on the floor.  The resident raised his/her voice and demanded the AP clean up the spill.  The AP was observed by staff to shake his/her finger to the resident and say “I’m going to lose my job if you hit me today.”  The resident stood up and raised fists in front of his/her body.  Another staff instructed the AP to step away from the resident, and offered to clean up the spill.  The AP didn’t move, the resident swung his/her arms toward to the AP, and the AP slapped the resident once with an open hand to the face.  The resident continued to yell at the AP.  The staff who had witnessed the AP strike the resident intervened and the AP was removed from the area.  Staff assessed the resident, the resident had a red mark on his/her face, but did not require any first aid or medical care.  The resident did not have a change in mood after the incident.

When the resident was interviewed, s/he was able to recall the events of the incident.  The facility terminated the AP’s employment.  The AP did not respond to requests for an interview.

Augustana Healthcare Center Minneapolis Complaint Findings for Financial Exploitation

In a report concluded on April 23, 2013, the Minnesota Department of Health cites Augustana Residential Care Minneapolis for exploitation by staff.

It is alleged that financial exploitation occurred when the alleged perpetrator (AP) took gift cards from a client’s apartment without permission and was observed on a video redeeming the gift cards at a Target store.

Substantiated Complaint of Financial Exploitation Against Augustana Residential Care Minneapolis

Based on a preponderance of evidence, the allegation that a client was financially exploited by the AP is substantiated.  The AP worked for the client in the capacity of a Home Health Aide (HHA) for three months and had access to the client’s dresser.  The AP was the last person in client’s room on the date that the gift cards were found to be missing.  The client had a toltal of five Target gift cards valued at $15.00 each.  All five of the gift cards were missing from the client’s dresser.

Attempts were made to interview the AP including phone message and subpoena.  The AP did not present herself for the interview.
