Agewell Home Care Edina Complaint Findings for Neglect

In a report concluded on January 31, 2013, the MDH cited Agewell Home Care Edina for neglect of health care.

The allegation of neglect is based on the following: A client was not provided with a timely medical evaluation after a fall.  In addition when she was transferred to an emergency department, information regarding her health care directives was not sent.

Note: the facility disputes this finding, check with the MDH for most current information.

Agewell Home Care Edina Complaint Findings for Nursing Care

In another report concluded on March 18, 2011, the MDH cited Agewell Home Care Edina for nursing care.

The allegation of inadequate nursing care is based on the following:  On the night of 9/10/2010, Client #1 fell while being walked by Employee #2.  Employee #2 did not report the fall as is required by policy.

Agewell Home Care Edina Complaint Findings for Patient Rights

In a third report concluded on March 16, 2011, the MDH cited Agewell Home Care Edina for patient rights violations.

The allegation of a patient rights violation is based on the following: Client #1 was not informed of the staff’s schedule for 12/24/2010.

Agewell Home Care Edina Complaint Findings for Neglect – Falls

In a fourth report concluded on March 11, 2011, the MDH cited Agewell Home Care, Edina for neglect of health care – falls.

The allegation of neglect is based on the following: A client had a fall with injuries when he was left unsupervised in his room.   The client’s service plan indicated he was never to left unattended.
